1. ELIGIBILITY:

This Promotion is open to legal residents of the United States or Puerto Rico, 18 years of age or older, who reside, at the time of entry, in The District of Columbia, Puerto Rico or in one of the following states; Alabama, Arizona, California, , Florida, Georgia, Illinois, Louisiana, Maryland, Mississippi, Missouri, Nevada, New Jersey, New Mexico, North Carolina, Pennsylvania, South Carolina, Texas, Virginia, or Wisconsin. Employees of Anna’s Linens, Inc. (the “Sponsor”) and their affiliates subsidiaries, vendors and agencies, as well as their immediate family members (spouse, parents,children and siblings, and their respective spouses) and persons living in the household of each, are not eligible to participate. All federal, state and local laws and regulations apply. Void outside of the United States, Puerto Rico and/or wherever else prohibited or restricted by law.

2. HOW TO ENTER:

Beginning on/about 4/15/13 and ending on 6/30/13 (the “Promotion Period”), specially-marked cash register receipts issued at participating Anna’s Linens locations will have instructions printed on them that will invite participants to visit a Sponsor-hosted website or call a toll-free telephone number, and answer customer satisfaction questions in the form of a confidential survey. Answers to the survey will have no effect on an entrant’s ability to win a prize. Participants may complete and submit a survey as often as they visit an Anna’s Linens location, but will receive a maximum of one (1) Sweepstakes entry per week. All information received becomes the property of the Sponsor, and will not be shared with or sold to any third party.

3. AMOE (Alternate Means of Entry):

To enter without making a purchase at an Anna’s Linens location, or without completing the survey, hand-print your name, complete address, city, state, zip code, daytime phone number, birth date, and e-mail address (optional) on a 3” x 5” card and mail it to: Anna’s Linens Customer Satisfaction Survey Sweepstakes, C/o USSC, 625 Panorama Trail (Suite 2100), Rochester, NY 14625. Mailed entries will be entered into a Drawing Period, based on the date the entry is received. No copies, facsimiles or mechanical reproductions of any kind will be accepted. Limit one mail-in entry per postmarked envelope. All entries received become the sole property of the Sponsor and will not be returned, nor is the Sponsor responsible for illegible, incomplete, late, lost, mutilated, misdirected, postage-due or stolen entries.

4. RANDOM DRAWING:

One (1) winner will be selected for each week of the Promotion Period (11 winners total) in a series of random drawings, from among all eligible entries received. Drawings will be held every 4 weeks, within fifteen (15) days of the last day of the fourth week in each Drawing Period. Entries will not be “rolled over” into subsequent Drawing Periods. The drawings will be conducted by an Independent Sweepstakes Administrator (the “ISA”), on behalf of the Sponsor.

5. PRIZES/ODDS:

One (1) $50 Anna’s Linens Gift Card will be awarded to each of the 11 winners.. Odds of winning will depend upon the total number of eligible entries received during each Drawing Period. Cards will be mailed via First Class USPS within 6 weeks of each drawing to the address provided by the entrant on their respective entry form. Gift Cards are redeemable for future purchases in any participating Anna’s Linens retail location. Prize is not redeemable for cash. Purchases may be deducted from the Gift Card until the balance reaches zero. The card does not have an expiration date and is subject to terms contained on the Gift Card. Limit one (1) prize per household during the Promotion Period. All taxes, if applicable, and all other costs associated with acceptance or use of the prize, not specified herein, are the sole responsibility of the winner. Total value of all prizes to be awarded is $550.

6. GENERAL TERMS/CONDITIONS:

Decisions of the ISA are final in all matters relating to this Sweepstakes. By participating, entrants accept and agree to abide by and be bound by these Official Rules. Sponsor is not responsible for any entries not received in time for each random drawing, Internet transmission errors, telephone line disruptions, printing or other mechanical/electronic errors, scanner or computer hardware or software malfunction, or any technical or human failures, errors or difficulties of any kind, or any unauthorized access to, or theft of entries at any point in the operation of this promotion. If such error occurs, Sponsor may, in its sole discretion terminate or modify the promotion, and select winners from all entries received up to the time of termination or modification. In the event of a dispute, online entries will be deemed to have been submitted by the Authorized Account Holder of the email address submitted at the time of entry and he/she must comply with these Official Rules. “Authorized Account Holder” means the natural person who is assigned to an email address by an Internet access provider, online service provider, or other organization that is responsible for assigning email addresses for the domain associated with the submitted email address. In no event will more than one (1) Gift Card be awarded weekly. Acceptance of prize offered constitutes winner’s permission for Sponsor to use their name and/or likeness for purposes of advertising and trade without further compensation, except where prohibited by law. By accepting prize, winner agrees to hold Sponsor and all those affiliated with this Sweepstakes, harmless against any and all claims and liability arising out of use/misuse of prize and entry/participation in the Sweepstakes. CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON(S) RESPONSIBLE FOR THE ATTEMPT, TO THE FULLEST EXTENT PERMITTED BY LAW.

7. WINNER LIST:

For the names of winners, send a stamped, self-addressed to: Anna’s Linens Customer Satisfaction Survey Sweepstakes Winner Confirmation Request, C/o USSC, P.O. Box 25497, Rochester, NY 14625. Sponsor must receive all requests for the names of winners by 30 days after published end date of promotion.
